Full-Time - Days - 40 Hours/Week

Primary Function:
Reporting to the Home Health Care Clinical Manager or the Associate Director of Hospice, the Team Clinical Coordinator – Home Health Care works collaboratively with the Home Health Care Clinical Managers and all patient care providers. The Team Clinical Coordinator – Home Health Care will provide a clinical resource and a support role for staff nurses. He/she functions within the professional nursing law and according to professional standards and level of knowledge and experience. In the absence of the Home Health Care Clinical Manager, the Team Clinical Coordinator – Home Health Care assumes responsibility for team management.

Qualifications

Education:
  • BSN required.

Experience:
  • 3+ years of nursing experience, 1 of which must be home care experience with an emphasis on specialty area of practice (as needed for specialty teams: hospice/heart failure/IV).

Licensure, Certification & Registration Requirements:
  • Registered Nurse in the Commonwealth of Pennsylvania.
  • BLS certification through the American Heart Association required.
  • Valid driver's license.

Other Requirements:
  • Knowledgeable of the professional nursing law and the Standards of Nursing Practice.
  • Leadership skills.
  • Current auto insurance with proof of coverage required. Must possess a driving record free from major violations within the past five years, and maintain an acceptable driving record with no major violations in accordance with Motor Vehicle Regulations and Abington – Jefferson Health's auto insurance policy. Any and all major violations incurred must be immediately reported to the manager, department head or Human Resources department.
