Primary Function:
Reporting to the Supervisor of Utilization Review, the Senior Social Worker helps patients/families cope with the multiple physical, social and economic adjustments they may encounter as a result of medical care or illness in order to optimize patient outcomes. The Senior Social Worker works in collaboration with the Care Coordination Manager as well as the broader interdisciplinary care team. The Senior Social Worker works jointly with the Care Coordination Manager to develop programs for patient populations as needed. Provides on-call services to assist with patient needs identified after departmental hours of operation. Provides care for patients of all ages.

Qualifications:
  • Experience: 2+ years of hospital-based medical social work experience or healthcare-related experience preferred.
  • Education & Training: MSW.
  • Other Requirements: Excellent communication and patient service skills required. Knowledge of community resources. Excellent time-management and task-prioritization skills required. Must be able to be self-directed and independent in structuring work processes. Competent computer skills. Ability to participate in evening, weekend and holiday on-call and on-site coverage rotation.

Conditions of Employment:
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI fingerprint-based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
