Senior Social Worker, MSW

Full-Time - 8:30am-5pm - Days, Monday-Friday with Weekend & Holiday Coverage, as well as Evening Call - 40 Hours/Week

Responsibilities:
Under direct supervision of the Director or Supervisor of Case Management, assists inpatients and Abington – Lansdale Hospital outpatients and their families (from infancy through late adulthood) with psychosocial problems that contribute to or are influenced by impediments to health and the adjustments necessary to medical care, and helps them deal with these problems in such a way that they may realize their fullest possible physical and social functioning. The social worker performs assessment, planning implementation, facilitation and evaluation of discharge plans for patients requiring referral to another institution or community services. The social worker may work with a case manager as part of the interdisciplinary team. The Social Worker works collaboratively with the hospital’s healthcare team and acts as patient/family liaisons with hospital services, managed care carriers and community service providers. Provides “on call” services to assist with patient’s community needs after hours.

Qualifications:
  • At least three (3) years of recent experience in medical/surgical, discharge planning, UR or home care preferred.
  • Master's degree in Social Work from a graduate school accredited by the Council of Social Work Education required.
  • Advanced communication and interpersonal skills with all levels of internal/external customers.

Conditions of Employment:
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI fingerprint-based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
