Senior Social Worker, Hospice
Home Health Care Hospice - NWG
Part-Time - 8:30am-5pm - Days, Variable to Accommodate Part-Time Employment - 20 Hours/Week

Responsibilities:
Under the direction of the Medical Social Work Supervisor, assists patients/families with psychosocial problems that contribute to or are influenced by impairment of health, physical handicap and the adjustments necessary in medical care, and helps them deal with these problems in such a way that they may realize their fullest possible physical and social functioning. The Home Care Social Worker will provide service to clients of all ages.

Qualifications:
  • Minimum 1-2 years of experience in a related healthcare agency is required.
  • Home care experience preferred, but not required.
  • Master's degree in Social Work from a graduate school accredited by the Council on Social Work Education.
  • Thorough knowledge of the principles and practices of social service with ability to plan, organize, instruct and coordinate.
  • Ability to communicate and coordinate efforts with other health disciplines for enhancement of health delivery services to patients.
  • This position follows the driving policy of Abington – Jefferson Health which requires one to have a valid driver’s license and reliable transportation. Current auto insurance with proof of coverage required. Must possess a driving record free from major violations within the past five years, and maintain an acceptable driving record with no major violations in accordance with Motor Vehicle Regulations and Abington – Jefferson’s Health auto insurance policy.

Conditions of Employment:
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI fingerprint-based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
