Reporting to the Home Healtn Care Clinical Manager, the Staff Nurse – Home Care is responsible for the assessment, planning, implementation, evaluation and documentation for assigned Home Health Care patients. Appraises individual and family health needs and plans for coordinated home health services for clients of all ages. Functions according to the Professional Nursing Law, according to professional standards, and according to their level of knowledge and experience.
This position is responsible for telephonic triage and conducting in-home urgent patient visits between the hours of 5:00pm – 8:30am. Service area includes various areas of Montgomery, Bucks, and Philadelphia Counties. This is a salaried (exempt) position that is based on 80 hours/biweekly. However, any hours worked over 80 will be compensated using a bonus structure.
Graduate of an approved school of nursing.
Minimum 1 year of inpatient med/surg experience is required. Home Health Care experience preferred, but not required.
Current registered nurse licensure in Pennsylvania. Valid driver's license. BLS certification through the American Heart Association is required.
Satisfactory FBI Fingerprinting and Child Abuse Clearances dated within 5 years.
Knowledgeable of the professional nursing law and the Standards of Nursing Practice. Current auto insurance with proof of coverage required. Must possess a driving record free from major violations within the past five years, and maintain an acceptable driving record with no major violations in accordance with Motor Vehicle Regulations and Abington – Jefferson Health's auto insurance policy. Any and all major violations incurred must be immediately reported to the manager, department head or Human Resources department.
