Under the direction of the Home Health Nursing/Hospice Supervisor, the incumbent is responsible for the assessment, planning, implementation, evaluation and documentation for assigned Hospice patients. He/she appraises individual and family health needs and plans for coordinated nursing services for clients of all ages. The Hospice nurse functions according to the Professional Nursing Law, according to professional standards, and according to his/her level of knowledge and experience.
Conditions of Employment:
Minimum one to two years of med/surg experience is required.
Home Health Care/Hospice experience preferred, but not required.
End-of-life experience highly desired, but not required.
Graduate of an approved school of nursing.
Current professional nurse license in Pennsylvania or eligible for licensure in Pennsylvania.
Valid driver’s license.
CPR certification through the American Heart Association.
Certification as Hospice and Palliative Care Nurse preferred.
Must have knowledge of the professional nursing law and the Standards of Nursing Practice.
This position follows the driving policy of Abington – Jefferson Health which requires one to have a valid driver’s license and reliable transportation. Current auto insurance with proof of coverage required. Must possess a driving record free from major violations within the past five years, and maintain an acceptable driving record with no major violations in accordance with Motor Vehicle Regulations and Abington – Jefferson’s Health auto insurance policy.
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI fingerprint-based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
