Responsibilities:
Reporting to the Associate Director of Home Health Care, the Physical Therapist – Per Visit consults, evaluates and provides professional physical and rehabilitative therapy to patients of all ages in their homes as prescribed by the physician. Treatments are carried out to restore function, prevent disability and/or decrease pain. The Physical Therapist – Per Visit is responsible for maintaining high standards of care as established by the profession and according to his/her level of knowledge and experience.

Qualifications

Experience:
  • Minimum 2 years of recent clinical physical therapy experience is required.

Education & Training:
  • Bachelor's, Master's or Doctorate degree in Physical Therapy from an accredited college or university required.

License, Certification & Registration:
  • Current professional Physical Therapist license in Pennsylvania.
  • Valid driver's license.
  • CPR certification required.

Other Requirements:
  • Knowledge of the professional standards of practice, Medicare Conditions of Participation, and legal, regulatory and insurance requirements.
  • Strong communication, organizational and clinical skills.
  • Current auto insurance with proof of coverage required.
  • Must possess a driving record free from major violations within the past five years, and maintain an acceptable driving record with no major violations in accordance with Motor Vehicle Regulations and Abington – Jefferson Health's auto insurance policy. Any and all major violations incurred must be immediately reported to the manager, department head or Human Resources department.
