Medical Assistant II
Comprehensive Primary Care Plus
Full-Time - Days - 40 Hours/Week

Primary Function:
The Medical Assistant Navigator (MAN) reports to the Director – PCMH Program and will function across various settings as a liaison between patients and members of the healthcare team. In collaboration with the patient's care team, the MAN is responsible for the telephonic and/or face-to-face coaching, counseling and consultation for patients managing chronic health conditions. The MAN will assist patients with basic health education regarding their medical conditions, assist patients in setting and meeting goals to improve their health, help with medication management, etc. They will also help the patients navigate healthcare systems, while providing support though regular phone calls and during office visits.

Qualifications

Experience:
  • 3+ years of experience in health or social service field required.
  • Health coach experience preferred.

Education:
  • High school diploma or GED.
  • Graduate from a Medical Assistant program required.

Licenses/Certification Requirements:
  • Medical Assistant certification required.
  • Valid driver's license.
  • Proficient and knowledgeable within Epic. Utilize and remain proficient with EMR as it relates to position responsibilities.
  • Demonstrated ability to use computers for data entry, including Access and Excel.
  • Experience with population health databases and email communication is essential.

Conditions of Employment:
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI fingerprint-based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
