Primary Function:
Under the direction of the Home Health Care Clinical Manager of the Patient Flow Center or the Hospice Associate Director, the Home Care/Hospice Nurse Liaison collaborates with the host hospital case management department to accept referrals. He/she assesses the need for home care or hospice services consistent with admission policies and consistent with CMS regulations. Complies with all policies and procedures of host hospital and Jefferson Health Home Care and Hospice, and exhibits professional behavior consistent with the service standards. The Home Care/Hospice Nurse Liaison accepts referrals for patients of all ages.

Qualifications

Education:
  • BSN required.

Experience:
  • 3+ years of recent home care/hospice nursing experience is required.

Licensure, Certification & Registration Requirements:
  • Registered Nurse in the Commonwealth of Pennsylvania. Maintains current licensure.
  • Valid driver's license.

Other Requirements:
  • Computer skills required, including familiarity with computer software in AJH system.
  • Current auto insurance with proof of coverage required. Must possess a driving record free from major violations within the past five years, and maintain an acceptable driving record with no major violations in accordance with Motor Vehicle Regulations and Abington – Jefferson Health's auto insurance policy. Any and all major violations incurred must be immediately reported to the manager, department head or Human Resources department.
