Care Coordinator Manager, RN - Abington - Jefferson Medical Group

Primary Function:
Under the general direction of the Director of Care Coordination, the Care Coordinator Manager will assist the director in mentoring, onboarding and managing day-to-day operations of the care coordination team, in addition to supporting practices and providers as a care coordinator.

Essential Functions:
  • Coordinates the care for the practices' high-risk patients served to promote preventive/wellness/education for high-risk patients with chronic diseases in the outpatient environment to meet the Jefferson enterprise care coordination goals and metrics.
  • Coordinates the smooth transition of care for cohort of high-risk patients between inpatient and outpatient arenas in order to prevent readmission to the hospital.
  • Assists the director in monitoring, evaluating and improving the efficiency and effectiveness of the care coordination program in the achievement of health, experience of care and cost outcomes through managing a team of the care coordinators and monitoring documentation, productivity and workflow.
  • Assists the director in chart audits to identify best practices and opportunities for improvement in the metrics and outcomes. Creates and monitors plans for improvement (PDSA) to meet the system goals for care coordination.
  • Optimizes training of care coordination team from onboarding to new projects and processes.
  • Is a patient advocate, working closely and collaboratively with internal/external resources to ensure the needs of our patients are consistently met.
  • Interacts with co-workers, visitors and other staff consistent with the values of Jefferson.
  • Performs other related duties as assigned.

Qualifications

Educational/Training Requirements:
  • Education and Training:
    • Graduate of an approved school of nursing. Bachelor of Science degree in nursing or related degree required
  • Other Requirements:
    • Demonstrated competence in leadership and patient education.
    • Excellence in verbal and written communication skills.
    • Must meet requirements of PA Act 73 regarding child abuse clearance.
    • Excellent communication and organizational skills.

Certificates, Licenses and Registration:
  • Registered Nurse in the Commonwealth of Pennsylvania.

Experience Requirements:
  • Experience: 5+ years of experience in ambulatory care management/coordination within PCMH, CPC+ or other value-based care models.

Additional Information:
  • Knowledge of medical terminology.
  • Familiarity with medical office procedures and flow of electronic medical record.
