Care Coordination Manager, RN
Care Coordination Management - Social Work
Full-Time - 8am-4:30pm - Will Work Predominantly Monday-Friday but Will Have Weekend, Holiday & Evening Call Obligations - 40 Hours/Week

Responsibilities:
Under the supervision of the Associate Director of Care Coordination Management, the Care Coordination Manager performs assessment and evaluation of patient symptoms, past medical history and plan of care for inpatient, SPU and observation status patients requiring medical, surgical and maternal and child healthcare to assist in level-of-care determinations. The Care Coordination Manager will be knowledgeable concerning regulatory standards and will assist with keeping the Care Coordination Management Department compliant with these standards. The Care Coordination Manager also provides interventions to ensure safe and effective care transitions of the hospitalized patient to the next level of care. This will include, but is not limited to, transitions between inpatient care, rehabilitation facilities, skilled nursing facilities, assisted living facilities, outpatient programs and home health and hospice agencies.

Qualifications:
  • 2-plus years of medical/surgical experience required.
  • 2-plus years in home care and/or utilization review preferred.
  • BSN required.
  • Current licensure as Registered Nurse in the Commonwealth of Pennsylvania required.
  • Case Management Certification preferred.
  • Must have excellent communication skills and patient service skills.
  • Knowledge of community resources helpful.
  • Good time management and task-prioritization skills.
  • Self-direction and independence in structuring work process.
  • Basic computer competency skills required.
