Under the supervision of the Associate Director of Care Management, the Care Coordination Manager performs 2 primary functions for patients across the full spectrum of age. The assessment and evaluation of patients' documented symptoms, past medical history and plan of care to ascertain the most appropriate level of care for patients requiring inpatient, SPU and observation services. The Care Coordination Manager also provides oversight to and interventions for the safe and effective transition of the hospitalized patient to the most appropriate post-acute-level care available to meet their ongoing medical needs. The Care Coordination Manager will perform these functions in compliance with all regulatory guidelines.
Conditions of Employment:
Education & Training: Graduate of an accredited school of nursing. BSN required for new hires and transfers effective July 1, 2017.
Experience: 2-plus years of medical/surgical experience required. 2-plus years in home care and/or utilization review preferred.
License, Certification & Registration: Current licensure as Registered Nurse in Commonwealth of Pennsylvania. Case Management Certification preferred.
Other Requirements: Excellent communication skills and patient service skills. Knowledge of regulatory standards and community resources. Good time management and task-prioritization skills. Self-direction and independence in structuring work process. Basic computer competency.
Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI fingerprint-based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
