Responsibilities:
Assumes the primary responsibility and accountability for the provision of safe and effective drug therapy and other pharmacy care services intended to achieve outcomes related to the cure, diagnosis or prevention of disease; elimination or reduction of patient’s symptoms; arresting or slowing of a disease process; and improving a patient’s quality of life in collaboration with a multidisciplinary team. Provides oversight, coordination and/or support of clinical programs under the general direction of the Assistant Director of Pharmacy. Patient population ranges in age from infancy through late adulthood.

Qualifications:
  • 3+ years of clinical experience and development required, if PGY1 and/or PGY2 residency not completed.
  • BS in Pharmacy from an ACPE-accredited college of pharmacy required; PharmD preferred.
  • Completion of ASHP-accredited PGY1 residency and/or PGY2 preferred, or 3+ years of clinical experience and development required in lieu of formal residency training.
  • Current Pennsylvania Pharmacist licensure or eligible to complete the licensure process within 90 days of employment.
  • Board certification in area of expertise or pharmacotherapy is required within 18 months of employment.
  • Excellent written and verbal skills.
  • Proficient with computer database and spreadsheet programs.
  • Excellent preceptorship and mentorship skills.
  • Proficient with hospital computer systems and medication ordering systems.
  • Member of the American Society of Health-System Pharmacists (ASHP); active membership in other local, state or national professional organizations is preferred.
